Targeting the Myofascial Injection Site for Treatment of Achilles Tendinosis by Measuring the Hand Span.

Background: Achilles tendinosis refers to a degenerative process of the tendon without histologic or clinical signs of intra-tendinous inflammation. It is common to inject into the myofascia between the medial/lateral gastrocnemius and the soleus muscle, rather than into the tendon or muscles directly. To save time determining the proper placement of the ultrasound probe, the length of the Achilles tendon and leg were compared to the patient¡¯s hand span.

Methods: The length between the musculotendinous junction and the calcaneus in an erect position was checked in 50 male and female patients aged between 20 and 80 years, along with their height and hand spans.

Results: The mean age was 45.6 ¡¾ 10.5 cm in men and 46.6 ¡¾ 10.5 cm in women. The mean height was 168.5 ¡¾ 4.5 and 160.3 ¡¾ 3 in male and female patients, respectively. The mean leg length was 44.3 ¡¾ 2.5 and 38.2 ¡¾ 1.9 cm in male and female patients, respectively. The mean length of the Achilles tendon was 23.7 ¡¾ 1.1 and 20.3 ¡¾ 0.9 cm in men and in women, respectively. The mean hand span was 21.8 ¡¾ 0.8 and 21.0 ¡¾ 1.3 cm in men and in women, respectively.

Conclusion: The length of the Achilles tendon was very similar to their hand span. Therefore, the targeted myofascia injection between the medial/lateral gastrocnemius and the soleus muscle is 2-3 fingerbreadths (3.8-5.7 cm) above 1 patient hand span from the calcaneus in an erect position.
