Perineural injection of Botulinum toxin A for Postherpetic Neuralgia : Case Report

Abstract
Background:
Postherpetic neuralgia can be a debilitating complication of herpes zoster, that is defined as persisting pain past 3 months after herpes zoster infection. Conventional therapies currently applied in clinical practice may fail to provide adequate analgesic treatment. Botuliunum toxin A(BTX-A) decreases muscle contraction and hypertonia and there is currently an increasing interest in the role of BTX-A for neuropathic pain.
Case report
We present a case of a 70 year old man with severe pain from post herpetic neuralgia that was refractory to the conventional treatments. Perineural injection of BTX-A was performed for the affected dermatome. The injection proved effective in decreasing pain and allowing reduction of pain medication that persisted at the 3 month follow up.
Conclusion
Reduction of pain in this case suggests that perineural injection of BTX-A may be an effective alternative to treatment of pain from postherpetic neuralgia.
